Leadership had a documented safety plan for the first time, and zero formal incidents in the following quarter.

The situation

A mid-sized agency was seeing repeated incidents involving distressed clients — escalating confrontations, staff intimidation, and two formal incident reports in six months. There was no formal safety program, and staff reported feeling unprepared and unsupported.

The engagement

A two-day Risk Discovery: interviewing frontline staff, reviewing incident reports, assessing the physical environment. We identified three high-priority areas and one significant gap in incident documentation. Training followed in two half-day sessions.
